box-sizing: border-box;'><span leaf="">据了解，本次评选围绕全省医疗机构门急诊处方、住院患者抗菌药物使用情况、使用强度及分级管理等核心指标展开，综合评估数据上报的完整性、准确性及临床改进成效。渭南市中心医院在2025年度监测数据中表现突出，反映出其在抗菌药物精细化管理方面的扎实工作，也为全省合理用药提供了重要参考。</span></p><p style='text-indent: 2.125em; box-sizing: border-box;'><span leaf="">医院自2012年加入全国抗菌药物临床应用监测网以来，已连续14年高质量完成监测数据上报工作。14年来，逐步建立起由药剂科牵头，医务科、感染控制科及临床科室共同参与的“四级质控体系”，形成覆盖全流程的管理模式。药剂科每月对门急诊处方和住院医嘱中的抗菌药物使用情况进行专项点评与指标管控，动态监测使用趋势，并及时向临床科室反馈。临床药师深入病房参与感染疾病会诊，协助制定个体化用药方案，监测并处理不良反应。此外，依托医院信息系统与合理用药软件，实现了抗菌药物分级管理规则的自动拦截功能，有效防范超权限、超剂量用药。</span></p><p style='text-indent: 2.125em;
